1. INTRODUCTION.                                                                                                 ----------------------------

Lucam Services provides and develops services which meets professional and legislative requirements within our field to facilitate the safe, effective and appropriate usage of medicines, dressings and appliances, their storage, stock control or disposal, and associated record keeping in Care Homes and other establishments.

2. PROVISION OF SERVICES.                                                                               ---------------------------------------------

The Pharmaceutical services Provision for Homes can be commissioned on three levels, which include the following:-

2.1. LEVEL 1:

2.1.1. Providing advice and support on the administration, storage, disposal and record keeping for medicines, dressings and appliances

2.1.2. Input into development of policies and procedures relating to the safe handling of medicines in the home

2.2. LEVEL 2:

2.2.1. Contribute to training carers in the homes.

2.3. LEVEL 3:

2.3.1. Medication Review for Service Users

2.3.2. Care plans for use in the homes

4. EVALUATION.                                                                                                    -------------------------

Records will be kept of each visit made to the home and will include:

4.1. the date on which the visit was made 

4.2. names of individuals seen (service users, care staff and other health professionals) in line with data protection guidance                                                                      

4.3. clinical interventions made and advice given (if applicable)                                   

4.4. training of care staff

5. MONITORING.                                                                                                   -------------------------

Methods will be developed to enable monitoring of service provision by:-

5.1. analysis of visiting records and outcome of advice                                              

5.2. clinical interventions and training                                                                       

5.3. maintenance of a record book at the home in which patients and care staff can comment on the service provided

6. TRAINING and UP-DATES.                                                                                 -------------------------------------------

The Pharmacist will complete:

6.1. any relevant distance learning courses (to ensure up-to-date knowledge)              

6.2. any other necessary education and training as determined by the commissioning authority, under the terms of the contractual arrangement                                         

6.3. specialist training needed for specific areas of service
