Traditionally, the relationship between community pharmacist and care homes has always been based around the supply of medication. New national minimum standards applied to care homes under section 23 of the Care Standards Act 2000, impact on the home's facilities and services. One of the underpinning themes is that all homes must have a quality work-force consisting of competent, well-trained managers and staff, as this is fundamental to achieving good quality care for service users. The standards stipulate that the home must achieve a minimum ratio of 50 percent staff trained to National Vocational Qualification (NVQ) Level 2 or equivalent. This excludes registered managers and, in homes providing nursing care, excludes registered nurses. A staff training and development programme must be in place that meets National Training Organisation workforce training targets. The National Care Standards Commission (NCSC), ensures the safety of the individual in the homes. We can help provide the underpinning knowledge that the students require to complete the NVQ. In addition the legislation has focused the attention of the care homes on the need to provide regular staff training, again an area we are already involved in. The NVQ that most care staff study for is the NVQ in Care Level 2. This is split into mandatory and optional units to allow for differing needs in different care settings. The NVQ does not have a unit dealing solely with medicines administration but there are several areas that cover its essentials, including patients' rights, effective communication, signs and symptoms, emergencies, reporting and recording, and awareness of health issues. We can provide excellent and relevant advice on all these topics. Most Care Home Managers feel that the omission from the NVQ of a section on medicines administration, dealing specifically with the mechanics of administration and the legislation surrounding it, leaves a vital gap in their staff training and are consequently keen to train their staff in this area. They are also keen to have a programme of on-going training that deals with relevant disease states, side-effects, danger signs and symptoms, etc, to try to increase staff awareness of their clients' medical conditions and the drugs used to treat them. Once staff have completed the training, the issuing of certificates is important. While a certificate is for attendance only, and does not imply that recipients have met NVQ requirements, it gives recognition of what they have achieved. It is also good for the home to be able to display the certificates as it demonstrates their commitment to staff training, and trained staff should be able to provide a better level of care.
